Outcomes of non-operative treatment in patients with penetrating abdominal traumas

Abstract

Introduction: Laparotomy has been considered the traditional treatment of the penetrating wounds to the abdomen; however, the validity of this opinion has been discussed at the present time.

Objective: To report the results of the non-operative conservative treatment of patients with penetrating abdominal wounds without initial criteria of laparotomy.

Material and Methods: An observational prospective study was performed on 115 patients with penetrating abdominal trauma. A total of 37 injured patients who were selected by inclusion criteria, underwent non-operative treatment according to a trauma management algorithm which includes admission protocol, observation and monitoring for the early detection of possible intra-abdominal lesions.

Results: The male sex and the third decade of the life prevailed in the study. During the admission and observation period, three injured patients presented evidence of intra-abdominal lesions, so laparotomy was required. Surgical intervention was not necessary in the remaining patients.

Conclusions: Conservative or non-operative treatment of penetrating abdominal trauma is a sure modality if the selection criteria and controlled observation are completed; this last one allowed the early diagnosis of the failure of conservative treatment, thus avoiding the occurrence of serious surgical complications.
