Survivance in the removal of non-small cell lung cancer. Validity in lymphadenectomy

Abstract

Introduction: Sampling and complete lymphadenectomy complement the surgical access to chest affections when standard or extended lymphadenectomies or simple exeresis are performed for biopsies. Its evaluation compared to patients who did not undergo mediastinal lymphadenectomy allows to define the management of patients with lung cancer.

Objective: To determine the survival rate in patients with resectable lung cancer lesions that underwent treatment with sample and complete lymphadenectomy compared to those without mediastinal lymphadenectomy.

Material and Methods: A longitudinal case series study was carried out in 118 patients with resectable non-­small cell lung cancer. All patients were subdivided into three groups. The 55 patients belonging to group A received treatment following sample lymphadenectomy (1996-2010), whereas the 45 patients belonging to group B received treatment with complete lymphadenectomy (2011-2019). The 18 patients belonging to group C did not receive mediastinal lymphadenectomy. Five-year survivals were analyzed using the Kaplan Meier statistics.

Results: Patients with complete lymphadenectomy had a survival rate of 56,8 %, those who underwent sampling lymphadenectomy had a survival rate of 40,8% and those with no mediastinal lymphadenectomy had 50,0 %.

Conclusions: There were no statistically significant differences in the survival rates in each of the three studied groups. Depending on the type of lymphadenectomy, in the study of pathological anatomy, the percentage of lymph nodes affected would allow for a better prognostic evaluation of survival for these cases.
