Sebaceous nevus of Jadassohn and its association with basal cell carcinoma

Abstract

Introduction: Sebaceous nevus of Jadassohn is a hamartoma of the pilosebaceous follicular unit characterized by hyperplasia of the epidermis, presence of immature hair follicles and sebaceous and apocrine glands. Between 10-50 % of patients with this type of nevus develop a variety of tumors that are usually benign (80 %) and rarely malignant (0-22 %).

Objective: To describe the diagnostic and therapeutic process of a patient with a sebaceous nevus of Jadassohn in which a basal cell carcinoma originated.

Case presentation: Sixty-seven-year-old female patient with a history of previous health who came to the dermatology office presenting a lesion on the scalp from birth. She reported growth of the lesion, change in color, and occasional bleeding during the last few months. After clinical and dermatoscopic examination, a sebaceous nevus of Jadassohn associated with basal cell carcinoma was suspected, so an excisional biopsy of the lesion was indicated. Histology confirmed the diagnosis; hence the patient underwent surgery, and is currently under clinical follow-up with no signs of lesion recurrence.

Conclusions: Sebaceous nevus of Jadassohn has a clinical course characterized by an elevated risk of developing benign and malignant skin tumors. Among the malignant neoplasms that can originate, basal cell carcinoma is the most frequent, so an adequate follow-up of these patients through periodic clinical and dermatoscopic examination is necessary to ensure the early diagnosis and timely surgical treatment of these tumors.
